Accepted Volunteer Resources
Please refer to the Welcome Packet sent to you in the mail and included below for detailed information about the Volunteer Summer program. Forms and additional information are provided here for your convenience.
Pre-departure Paperwork
AJWS will need to receive the following completed forms and documents by June 3:
- Physician Certification
- Participant Agreement and Waiver of Liability
- Health Insurance Worksheet
- Individual Financial Agreement- High School Participants
- Individual Financial Agreement- College-aged Participants
- Participant Commitment Statement
- Copy of your passport
- VISA requirements (for those going to Ghana, Uganda, India only)

Payments
Deposits must be received by April 7 and final payments must be received by June 3. Please refer to the following forms for payment instructions:
Invoices will be emailed to you in the middle of April and the middle of May. These invoices will include details about all contributions made toward your program fees.
Many past participants have successfully raised money to help offset program costs. AJWS's Fundraising Guide offers tips to help your efforts.
