All participants will receive a Welcome Packet in the mail upon acceptance to the program. Forms and additional information are provided here for your convenience.
Pre-departure Paperwork
All participants must complete and submit the following completed forms and documents:
- Physician Certification (PDF) - to be completed by your primary care physician and any mental health professional whom you see regularly
- Participant Agreement and Waiver of Liability (PDF)
- Health Insurance Worksheet (PDF)
- Individual Financial Agreement- High School Participants (PDF)
- Individual Financial Agreement- College-aged Participants (PDF)
- Participant Commitment Statement (PDF)
- Copy of your passport
- Visa requirements as necessary

Payments and donations toward your program fees can be made online.
