All participants must complete and submit the following completed forms and documents:

• Physician Certification (PDF) - to be completed by your primary care physician and any mental health professional whom you see regularly
• Participant Agreement and Waiver of Liability (PDF)
• Health Insurance Worksheet (PDF)
• Financial Agreement- High School Participants (PDF)
• Financial Agreement- College-aged Participants (PDF)
• Copy of your passport (all participants) and visa (India, Uganda, and Ghana participants only)

Payments

Payments and donations toward your program fees can be made online.
